ISE Faculty Mentor Interview Form - This form should be used by ISE juniors and seniors to document the required faculty mentor meeting.  Juniors and seniors must complete this process each academic year to be released for course registration.

Directed Individual Study & Undergraduate Research Form - This form is for ISE graduate and undergraduate students who would like to request to complete a Directed Individual Study (IE 4000 or IE 7000) or ISE undergraduate students who would like to request to participate in undergraduate research while receiving course credit (IE 4800).

CAPP Adjustment Request- This form is used when a course does not articulate correctly in CAPP from another university or community college. Also used when substituting one MSU course for another course in the ISE curriculum. This form must first be filled out and signed by the student, and then submitted to the ISE department secretary for signatures.

Request to Enroll in Off-Campus Course - This form is used when a student wants to take a course at another university or community college. This commonly used by students when taking courses during the summer while at home, or in any semester while on a co-op rotation.

ISE Policy on Undergraduates in Distance Sections-Signature Ready - This form is used when a student wants to take a distance course while they are away from campus working at an internship or co-op job.

Undergraduate Research Application - Undergraduate students in the Department of Industrial and Systems Engineering have the opportunity to work closely with ISE faculty on cutting edge research projects. Many undergraduate researchers work on a funded project with a faculty member, where others work on independent research projects supervised by a faculty mentor.

Accelerated BS/MBA Application - Use this form to apply for the accelerated BS/MBA Program.
